In-Home Services (Services In-home and full access of Youth Center)

Healthy Body Healthy Mind is dedicated to service individuals with developmental challenges. Our mission is to provide a holistic remedy to reach the youth/family goals.

In order for Healthy Body Healthy Mind to provide services the youth/family needs to be connected to a Care Management Organization (CMO) and referred to our agency.   If your child or family is in need of therapeutic or support services please call Perform Care at 1-877-652-7624.  Perform Care is available 24 hours a day/7 days a week.  (If you have an emergency please call 911)
